Il avait placé son règne sous le signe de la simplicité désirant avant tout une « Église pauvre, pour les pauvres ».
La vie du souverain pontife a été marquée par de nombreuses hospitalisations et opérations. En 2021, il est opéré du côlon. Souffrant d’importantes douleurs au genou, il utilise un fauteuil roulant à partir de 2022. Dès 1957, il avait subi une ablation partielle d’un poumon à la suite d’une pneumonie aiguë avec plusieurs kystes pulmonaires. Lors des célébrations de ses dix ans de pontificat, à la question « Que souhaite-t-il pour lui-même ? », il répondait : « Que le Seigneur soit clément avec moi. Faire le pape n’est pas un métier facile. Il n’est pas possible de faire des études pour ce métier. »
